The Focus is You
Day Habilitation offers individuals the opportunity to be more active and interactive in
the community. Day Habilitation Services provides individuals with disabilities an opportunity
to enhance their own levels of independence in an environment that is entirely person-centered within
an individual's local community. New experiences, challenges and responsibilities increase independence
and social interactions, allowing people to make better choices about their lives.
Our staff works with each individual to determine their likes and interests to develop a service plan.
There is no "program" to fit into. The services are developed around what the individual wants.
A person-centered approach is the key concept to our Day Hab services. Participants may choose to
volunteer at a local not-for-profit organization to develop work skills or because they like to help
others. Recreation activities serve to increase socialization and community awareness. Other activities
are used to increase independence in the community in areas such as shopping or ordering in a restaurant.
Plans are developed by everyone involved in the person's life. At least twice a year the service plan
is evaluated, and if needed, modified to ensure that the individual is receiving the services they want
and which will benefit them.
How Does It Work?

Eligibility Criteria and Funding
Day Habilitation Services are offered to individuals with a developmental disability, who are at
least 18 years old. Referrals can be made through an individual's service coordinator or family
members by contacting the Director of Habilitation Services. Funding through the individual's
developmental service office is necessary and assistance will be provided in obtaining it.
Scheduling is based on the needs of the participant.
Hours of Operation
Day Habilitation operates Monday through Friday year-round. Starting and ending times vary according
to the individual program, but generally range from 8:00 a.m. to 4:00 p.m.
Transportation
Transportation arrangements are available to individuals unable to use the city bus.
