These are questions that need to be answered before a smooth transition plan can be developed for any student leaving school and entering the adult world. Finding solutions for a student with disabilities can be equally, if not more challenging.

The Transition Assistance Program was created through a federal grant by the Department of Education to assist a student's transition from school to adult life. Our primary goal is to assist the student with their transition planning for adult life before they have completed school, whether it be to live, work or continue to learn within their local community.

Our staff works with the student, their family, the schools and committees of Special Education in order to meet the needs of the student.

Here are some examples of assistance, which can be obtained through this program:
  • Advocating at IEP meetings and assist with transition plan development
  • Referrals
  • Assistance with benefit application processes.
  • Arranging tours of local service providers for the student and family members.
  • Interfacing with families - at their convenience.
  • Providing training related to eligibility and funding for adult services.
Program Eligibility

The Transition Assistance Program is available at no cost to students with disabilities, 15-22 years of age. The student must have an Individualized Educational Plan (IEP) and be a resident of Cortland County or its contiguous counties.

Analysis of Program

A component of this program involves research regarding the benefits the students receive. In order to ensure the program's success and determine its continuation the students, families and educators are asked to complete surveys periodically throughout the time of their participation in the program. Information collected from the surveys is confidential and will be adhered to as set forth by the Family Educational Rights and Privacy Act. Results of the surveys will be used to determine satisfaction with the benefits of the program and transition planning.
