Description

Seeking self-motivated individual to provide services according to assigned contract(s) to assist eligible participants in preparing for employment by addressing any barriers. Responsible for assessing job-readiness skills, coordinating the development of the applicable employment plans with funding agency and providing comprehensive case management services throughout the participant’s enrollment. Responsible for coordinating with other involved agency staff as well as staff from other agencies to identify needs and make appropriate referrals. This position is responsible for development of community-based employment, placements, and ongoing supports for the individuals served. Responsible to assist with crisis situations related to health needs (illness, mental health, substance use) child protective services, and/or loss of childcare or transportation. Assist in the development of plans to avert such emergencies.

Successful candidates will have a Bachelor of Science in human services, psychology or related field and 1-2 years of job related experience preferred, but willing to accept a combination of education and experience.  Excellent time management and organization skills; familiar with computers and sound decision-making.  Must have a valid NYS driver license and a reliable vehicle.
