Service Coordination helps individuals with disabilities explore what they want
and need in life and assists in making it happen. A Service Coordinator works in partnership with you,
your family and other people important to you, to develop, implement and maintain your personal goals.
Your Service Coordinator assists you in attaining the highest quality of life to live as independently
and productively as possible. Working with you and your advocate, your Service Coordinator will assist you by:
- Obtaining supporting information for planning.
- Identifying valued outcomes or personal goals.
- Planing and coordinating the supports and services the individual needs and wants to live as they desire.
- Implementing the plan by linking and referring the participant to activities, supports and services.
- Overseeing the plan to your satisfaction.
A Service Coordinator is knowledgeable about the types of services that are offered in your local community
and will assist you in finding the services that meet your needs. They are also able to negotiate the paperwork
system to help determine if you are eligible to receive services.
